Our team of providers includes the patient’s physician, hospice medical director, nurses, hospice aides, pharmacists, social workers, chaplains and volunteers. Members of the team schedule visits according to the patient’s unique needs.
Serene Palliative Care & Hospice staff members are available every day, around the clock – including holidays – to answer questions by telephone and to make a visit.

About Hospice Coverage

Hospice is a specialized type of medical care, coupled with just the right kind of support for family and caregivers. Hospice is a team approach to caring for patients and their families. This unique care model works very well. In fact, it is not at all uncommon to see that a patient’s condition improves while on hospice care.
When a patient’s pain and symptoms are well managed, a patient’s physical, emotional and spiritual well being can be similarly enhanced. Serene Palliative Care & Hospice provides this care and support wherever the patient feels most comfortable. This may be in the patient’s own home, an assisted living facility, group home, nursing home-virtually anyplace the patient calls home.
With the patient’s and family’s direct input, Serene Palliative Care & Hospice will develop a plan of care personally tailored to make sure that the needs of both the patient and loved ones are met.

Beginning a discussion about hospice can be difficult. Yet one of the most common comments we have heard from patients over the years is: “I wish that I had known about hospice care sooner.” Patients that begin our hospice program earlier express more satisfaction with their care and with the hospice experience.
